The required documents include a fully accomplished Personal Data Sheet (CS Form 212, Revised 2017), which you can download from the provided link. Additionally, include your performance rating for the period immediately preceding this publication, an authenticated Certificate of Eligibility, a photocopy of your diploma, and a photocopy of your transcript of records. If you have any relevant training certificates, include those as well (limited to relevant Learning and Development availments).
Once your application is complete, you can submit it in two ways. You have the option to hand-deliver your application to the PFDA Central Office, ASD – Human Resources Division (3rd floor). Alternatively, you can email your application to careers.hrd@pfda.gov.ph.
