If you are applying for a promotion, you need to include a duly accomplished Personal Data Sheet (PDS) – 2017 revised form, your work experience, a copy of your Employment Certificates with actual duties and responsibilities (for applicants applying for Salary Grade 22 and higher), and copy of your Training Certificates.
