To ensure a smooth application process, please note the following guidelines. You can apply to one (1) vacancy only. Properly fill out the required forms in TYPEWRITTEN FORMAT, leaving no blanks (use N/A for not applicable items). The necessary forms include the Application Form for Promotion/Appointment (Annex D), a notarized CSC Form 212 or Personal Data Sheet with a latest passport-size picture (white background, business attire) (Annex E), and a Work Experience Sheet (Annex E.1).

Additionally, you must provide specific documentary requirements, such as your Transcript of Records for Bachelor’s and Master’s degrees (if applicable), CSC Eligibility (CSC-authenticated copy), Board/Bar Rating/Bar Membership (authenticated copy, as applicable), and PRC ID (renewed/updated) if your profession requires it.

Relevant certifications, trainings, and seminars attended (with indicated training hours), along with Certificates of Employment, official and certified duties and responsibilities per employer, and Certifications/Personnel Orders of current and previous designations should be included in your submission. For those with previous service records, please provide them as well.

Additional documents may be required during the assessment period, including NBI, Police, and Barangay clearances (original copies), Birth, and Marriage certificates (PSA copy), and a Certification of graduation from the School Registrar (original copy).

Keep in mind that if you’re appointed, a Medical Clearance/Certification of Medical Fitness will be needed before assuming duty.

Remember, your application’s assessment will be based solely on the available and submitted documents. Only applications received within the deadline will be considered, and non-compliance with submission requirements may result in disqualification from further assessment. To make it easier for the evaluation process, applicants are requested to convert all accomplished forms, such as the Application Form and Notarized Personal Data Sheet, along with supporting documents, into separate PDF files. For example, you can create files like Application Form.pdf, PDS.pdf, Training.pdf, Cert. of Employment.pdf, Cert of Eligibility.pdf, Transcript of Records.pdf, and so on.

Organize your documents within an Applicant Folder with your name as the folder/file name (e.g., JUAN DELA CRUZ). Then, create a sub-folder with the correct vacancy code of the position you are applying for (e.g., 20ARF0001). It is essential to verify the vacancy code for the position you wish to apply to, as discrepancies may affect the assessment of your application.

After organizing your documents, please attach the PDF copies of the forms and all relevant documents within the sub-folder. Once everything is ready, you can then email your Applicant Folder in Zipped form to hrrecruitment@sss.gov.ph. In the Subject line of your email, use the format: Your Name and Vacancy Code.

Please remember to print and keep two original hard copies of the notarized Personal Data Sheet with Work Experience Sheet, as they may be required for submission at a later stage.
